  • Exiting Prison & Custody: Coordinating safe post-release pathways for youth and adults leaving correctional facilities or secure forensic environments.
     

  • Exiting Hospital & Inpatient Care: Managing fast-tracked, intensive discharge planning to bridge the gap between healthcare systems and the community.

Specialisations & Service Intersections

  • The Justice & Youth Justice Systems: Partnering directly with Justice Liaison Officers, Disability Justice Coordinators, Correctional Case Managers, and YJ workers to align post-release care.
     

  • Forensic Mental Health Services: Managing specialised care alongside Forensicare clinical teams and institutional discharge staff for participants with forensic histories or forensic mental health care plans.
     

  • DFFH & Child Protection: Supporting children, young people, and families navigating statutory care alongside Disability Practice Advisors and CP case managers.
     

  • Complex Support Needs & MACNI: Collaborating with Complex Support Needs Planners and participating in intensive care-team delivery models for MACNI programmes.
     

  • Statutory Guardianship: Communicating directly and transparently with statutory guardians and the Office of the Public Advocate (OPA) under formal guardianship orders.
     

  • Complex Challenges: Bringing creativity and decisive action to help individuals experiencing complex trauma, psychosocial disabilities, Foetal Alcohol Spectrum Disorder (FASD), Acquired Brain Injuries (ABI), Alcohol and Other Drugs (AOD) dependencies, family violence, or those experiencing homelessness or navigating housing instability.
